2. Along with the application forms, you will need to supply the following:
   • Photograph
   • Non-refundable $75 fee
   • Brief autobiography
   • Official transcripts sent directly to AGTS by all post-secondary schools attended (download transcript request forms, to give to your current school.)
   • Print the Ministerial and Personal Recommendation Forms [ministerial | personal] (PDF)
