   • Photograph
   • Non-refundable $35 fee
   • Brief autobiography
   • Official transcripts sent directly to AGTS by all post-secondary schools attended (download transcript request forms, PDF, very small)
   • Print the Ministerial and Personal Recommendation Forms [ministerial | personal] (PDF)
